week 5:feedback:Hi Cynthia, Overall, you have done an decent job but you missed some required elements of the assignment.  Your script was well composed however, your model is incorrect.  As shown in the linked example in the Week 5 directions orhere, you need to demonstrate the connections between the cells.  As displayed, every cell is interconnected and each column is connected which is not factual.  Once the cells in one column are defined then they can be arrowed or connected to their appropriate counter in the column to the right, then you have a logic model that demonstrates how the community needs will be addressed.  The largest deduction in this effort was that you attempted to combine both of your action plans into a single model.  Poverty reduction and health care access are not related and each require their own model.  Please email me if you have any questions. Kind regards, Dr. Holmes  https://www.mediafire.com/file/36yict8hgub8fob/document.pdf/file

  • Your presentation must follow this template.
  • This presentation will be structured as used in real public health offices. Given that you used the CHANGE Action Guide reading as your guide, the CHANGE Template presentation will be used. Thus, you should speak for eight to 12 minutes using PowerPoint slides to illustrate key points.
• Create a PowerPoint presentation addressing each of the required elements.
• Generate and submit a working link/URL to a narrated video of the PowerPoint presentation.
• Utilize images, graphics, and charts to support narration and slide content.
• Show that the presentation and Screencast are compelling toward the proposed action plans.

Remember the following:

• You should generate the detail and substance for those points primarily through your speaking and elaborations.
• Do not read directly from the slides.
• Since you will not be giving this presentation in front of a live audience, you are asked to submit the file as a screencast. You may use the software you are most comfortable with (TechSmith Capture, ScreenPal, or PowerPoint). Refer to the Video section tab for more details.

Presentation

Your presentation should have at least 13 slides that include the following:

• Slide 1: Your title slide
• Slide 2: Overview slide
  • List your name and education background. Ignore the CHANGE content in the template.
• Slide 3: Overview of assessment and evaluation
  • Follow the prompts in the template.
• Slide 4: Introduction to CHANGE process
  • Build on the third suggestion, “CHANGE frames and understand the current status of community health,” and insert a representative photo of your community.
• Slide 5: Key benefits of CHANGE
  • Build on the second suggestion, “CHANGE offers suggestions and examples of policy and environmental change strategies,” and insert a second representative photo of your community.
• Slide 6: CHANGE process
  • Briefly discuss the five sectors you researched.
  • Describe your data collection process (do not include a screenshot or image of the CHANGE tool as it is not relevant to this assessment).
  • Share the time frame and the purpose of your assessment (noting it was for your Capstone course in the MPH Program is perfectly acceptable for stakeholders and will explain the rapid time frame).
  • List resources utilized and briefly explain why they were chosen.
• Slide 7: Community Team
  • List the community stakeholders. Since you worked alone, there is no team to highlight but rather key stakeholders you identified in Week 3 Target Audience Assignment that are critical to your proposed action plans.
• Slide 8: Data Gathering
  • Follow the prompts and build on your Week 3 Data Collection Methods assignment. The “share success stories, challenges, and humorous anecdotes” note is for your speaker notes only and is not something you would place on the slide.
• Slide 9: Method Example (change heading to Assessment Findings)
  • Highlight the strengths found in the community and areas for improvement.
  • You will want to draw attention to the two primary areas of concern.
• Slide 10: Photos
  • Add photos of things your community does well, and a photo of the highest priority found in your assessment.
• Slide 11: Presenting Findings (change this heading to Action Plan)
  • Provide a summary of your first proposed action plan.
• Slide 12: Duplicate the previous slide with your second proposed action plan.
• Slide 13: Next Steps
  • Follow the prompts except for bullet 4 (remove “Highlight some of the lessons learned and how the process can be improved next year”).
