For this assessment, you will analyze your healthcare process or issue in the context of your practicum site.

The problem (topic) is the lack of continuity of care between patients and their nursing staff by rotating different nurses through a home every week. This leads to a decreased ability to recognize critical changes, resulting in missed opportunities, which can delay care and interventions, thereby increasing patients’ risk of rehospitalization.

  • Describe the patient population of your practicum site. (we treat post-surgical ortho patients, CHF patients, wound care patients, and chronic illnesses like cancer)
